    Notice About Tenant Screening Reports

    Tenant screening reports from consumer reporting agencies are sometimes used to assist landlords in making rental decisions. In regards to such reports, we may use such reports by contacting the following:

    The law requires us to notify you if we do not lease or rent to you based on information in that report. You also have the right to dispute the accuracy of the information in the report directly with the reporting agency and to obtain a free report from such agency if we do not lease or rent to you based on such report. You are also entitled to receive one free report every 12 months from any nationwide specialty consumer reporting agency used by us, as well as a free credit report every 12 months from each of the nationwide consumer credit reporting companies. Equinox, Experian and TransUnion. You can request this free credit report through the website www.annualcreditreport.com. You may dispute the accuracy of any information about you that is contained in such report directly with the credit reporting agency.


    I warrant that all the statements above are true and that any false statements may be grounds for rejection of this application. I further represent that I am not renting a room or an apartment under any other name, nor have I ever been dispossessed from any apartment, nor am I now being dispossessed. I herby give my permission to conduct an investigative consumer report on me. I further authorize Owner, its agent(s), and/or Century 21 American Homes to conduct their credit inquiries. No representation or agreements by any third party are binding on Owner/Agent/Century 21 American Homes, unless included in an executed lease. Owner/Agent/Century 21 American Homes makes no guarantee regarding the status of this application or the availability of any apartment. I also confirm that I have read and understand the Tenant Screening Report Disclosure printed above. If a lease is approved and executed, this completed application form becomes part of that certain lease.

    I also understand that there is a $20 per adult, non-refundable application fee associated with this application.


    Required Application Documents


    In order to process your application, copies of ALL of the following documents must be submitted along with a completed application form. Incomplete applications or applications submitted without all of the below required documents, will not be accepted or processed.

    For Applicants and Guarantors please provide copies of:


    1. Government issued Photo ID

    2. Three forms of proof of income. Proof of income may be any of the following, but you must provide 3:
    a. Three most recent and consecutive pay stubs
    b. First and signature page of your most recent Federal Income Tax Return
    c. Most recent W2 and/or 1099 forms
    d. Three most recent and consecutive bank statements
    e. Employment letter on employer’s letterhead verifying:
    o. Length of employment o Current position held o Annual income f. Annual/Monthly Social Security/Pension/Retirement Statement
    g. Proof of any additional income (Spousal Support, Child Support, Government Assistance, Etc)

    If you do not have 3 of the above due to being self-employed or commission based please provide copies of:

    A signed letter from your accountant verifying:
    1. Type and length of business
    2. Current position held

    3. Annual income

    If you have a pet, please attach ALL of the following:

    1. A recent picture

    2. Most Recent Vaccination record

    3. ESA Documentation (If Applicable)

    Please furnish a non-refundable application fee of $20.00 per Applicant, Co-Applicant and Guarantor for a credit check processing fee. Acceptance of said fees is not to be construed as acceptance of tenancy application. The Landlord will in no event be bound, nor will possession be given, unless and until a lease executed by the landlord has been delivered to tenant. The applicant and his/her references must be satisfactory to the Landlord. No representations or agreement by agents, brokers, or others are binding on the Landlord unless included in the executed lease. I hereby warrant that all my representations set forth herein are true. I recognize that the truth of the information contained herein is essential. I further represent that I am not renting a room or an apartment under any other name, nor have I ever been dispossessed from any apartment, nor am I now being dispossessed and that I am over 18 years of age.

    I agree to the terms and understand that there is a $20 per adult non-refundable application fee.*
